Job Description

Location: Bramlings House, Newbury, RG14 2FP

Hours: Full-time, 35 hours per week, Monday to Friday

Working Pattern: Site-based, 5 days per week

Join A2Dominion as a Support Officer and play a key role in delivering person-centred support to individuals in supported housing, helping people with a range of needs to maintain stable tenancies, build independence and make a genuine difference to their day-to-day lives.

An enhanced DBS is required for this role.

Key Responsibilities
Assess referrals and engage with prospective clients using a person-centred approach. Key to this role will be liaising with health professionals, other agencies and colleagues as appropriate
Ensure that prospective and current clients are fully aware of the support available, their financial obligations, rights and responsibilities under their tenancy/occupation arrangements and all related procedures
Agree and review client Independent Living Plans and Risk Assessments on a regular basis, deliver contracted hours of support to achieve the required outcomes
Flexibly deliver the support identified in an Independent Living Plan through regular person-centred contact, taking follow up action and coordinating wraparound support if required from other agencies (e.g. Adult Social Care, Criminal Justice, Health services) and internal departments
Forge and maintain links with partner agencies and other key stakeholders as appropriate to ensure that good communication and co-operation is maintained
Report any concerns relating to safeguarding of vulnerable people using the correct procedures, attending case conferences and other meetings as required
Ensure that all relevant documentation and records are completed and submitted in an accurate and timely manner
Actively contribute to the collection of data to be used in statutory returns
Handle complaints and representations in accordance with A2Dominion s policy and undertake any necessary investigations
Work outside of standard office hours, as required, to deliver services, following lone working procedures

More About You
Educated to GCSE level or equivalent in Maths and English (Grades A - C)
A working knowledge and understanding of how to support people in achieving and maintaining independence
Housing or Support related qualification is desirable but not essential
Experience of providing support to diverse client groups, including individuals with mental health needs and/or substance misuse
Knowledge of housing or welfare benefits systems and awareness of safeguarding principles would be desirable
Strong organisational and IT skills, including ability to manage workload, meet targets, and use management information systems
Ability to network and build strong relationships with partner agencies, community and key stakeholders
Ability to communicate effectively at different levels and deal confidently with distressed, aggressive or challenging behaviour
Driving Licence and access to own transport
